Once your form is completed and filed, the Registry will provide the company with documents that confirm the filing.

There are two sets of notifications available for a Voluntary Dissolution.  One notification is for the Submitting Party and the second notification is for the Person Having Custody of the Dissolved Company's Records.  The method of receiving the documents must be selected for each party.

Note: Only one notification method is available. Once the documentation has been sent, the method selected cannot be changed. The documentation cannot be requested to be re-sent at a later date. Therefore, it is important to keep your documents in a secure location.

Select "Email to the following address" if you want an electronic copy of the documents
